My office is on the corner of West Kennedy Blvd. and Delaware Street, 8/10 of a mile east of Howard Ave. and 2 short blocks west of North Boulevard, just west of the University of Tampa. Look for a stand-alone brick building on the north side of Kennedy. Enter my office through the double glass doors facing Deleware then straight ahead through the old wooden door into my waiting room. If you see the "please do not disturb" sign on the closed inner door, this means I am still with a client and will be with you shortly. If you are a new client, please fill out the "Client Intake Form" you will find in the clipboard on the bookcase OR you can download this pdf version fill it out and bring it in with you. If you plan to use your insurance benefits for these services, here's some information that you'll find helpful in dealing with your insurance company.
Directions: From I-275 South-bound take Exit 45A "Downtown West - Ashley Dr." and go West (South) to Kennedy Blvd. Turn right and go a few blocks past University of Tampa, just past North Boulevard to Delaware where you will see "The Lotus Room," a stand-alone red brick building on the corner. If North-bound on I-275, take Exit 42 "Armenia/Howard," go south on Armenia to Kennedy, turn left, go 8/10 of a mile (just past the RR tracks), see Lotus Room on the left. Turn onto Delaware and park along either side of Delaware (park with the flow of traffic) or in the vacant lot directly across Delaware from my office entrance.
From Brandon, Riverview (as well as Welsey Chapel and all points north) take I-75 to I-4 West. Go about 5 miles to the "Downtown East/West" exit, follow "Downtown West - Ashley Dr." to Kennedy (see directions above). An alternative to the I-4 route is the Crosstown Expressway where you will take exit 4 toward Willow Ave., turn a slight left onto W. Cleveland St. then turn right onto S. Newport Ave. then right onto W. Kennedy Blvd. (it's not as confusing as it sounds, just follow the signs) and continue west past UT (see directions above).
